Abstract Cardiac resynchronization therapy is an essential tool for treating patients with heart failure in different types of dilatative cardiomyopathy bundle branch block. This technique wildly used significant benefits terms quality life and effort tolerability along optimized medical therapy. In spite its benefits, several factors may influence efficacy such as etiology, lead position, or device settings. some cases, the anatomical variance coronary sinus could create technical difficulties advancing left ventricular lead. case report describes a female patient presenting decompensated failure, known complicated sclerosis multiplex fibrous tissue sinus, which created critical obstruction leading to impossibility advance underlines importance appropriate imaging investigation optimal interventional approach these difficult cases.
